1. Click the Search by Provider Type button and click CONTINUE.

-- To Find Occupational Clinics key onto FRONTLINE PROVIDERS/Industrial Medical Clinics and click CONTINUE.  Click the Locate Providers within a County.  Then, choose the desired County and click “Find Providers Now”.

-- To Find Hospitals key onto General Hospital and click CONTINUE.
Click the Locate Providers within a County.  Then, choose the desired County and click “Find Providers Now"

-- To Find Urgent Care Facilities key onto Other Physicians and Health Professionals and click CONTINUE.  From the scroll, select Urgent Care Clinic and then CONTINUE.  Click the Locate Providers within a County.  Choose the desired County and click “Find Providers Now.
